To get started, simply email your date of birth (mm/dd/yyyy) and a photo of the front and back of your insurance card to info@bridgecitymassage.com.

You can expect to be contacted within 5 to 7 days with instructions on how to schedule your medical massage appointment.

Important:
Please do not schedule an appointment until you have received approval from our office. If a session is scheduled without prior approval, it will be cancelled.

It is the client’s responsibility to confirm their benefits before booking a medical massage session. While we strive to provide accurate benefit verification, fees and coverage may vary based on eligibility and benefits at the time of your appointment. Please note that this verification is an estimate and not a guarantee of payment or approval of medical necessity. If you are uncertain about your benefits, out-of-pocket payment will be required at the time of service.

At this time, we ONLY accept Regence Blue Cross Blue Shield of Oregon. If you are insured under any other Blue Cross Blue Shield banner, please contact us to be added to the waitlist.

Out-of-network Option

If we are out-of-network with your insurance provider, you can pay out-of-pocket at the time of service. Upon request, we can provide a super-bill as proof of service. You can then submit this super-bill to your insurance provider for potential reimbursement.

Did you know that we accept FSA and HSA cards?

Use your Flexible Spending Account or Health Savings Account for massage therapy, and upon request, we’ll provide a super-bill as proof of service.

Currently, we do not accept MVA or PIP claims. Reach out if you would like referrals for motor vehicle accident or personal injury prevention LMT’s in the area.
